Minimally Invasive Decompression alone effective for some patients with stenosis plus stable spondylolisthesis

Relieving pain is a multi-disciplinary endeavour. Better management of pain is far more than simple administration of an adequate opioid or a nerve block done in a jiffy. It entails regulatory reform, humane care of the sufferer, and importantly championing evidence-based practices. It is clear that we are not consistently meeting the challenge of optimal pain management for a variety of reasons, including lack of knowledge. The Indian Society for Study of Pain’s (ISSP) annual scientific meeting takes place at Mumbai in February 2014. In the previous years the well attended workshops and panel discussions were testament to the high quality discussions carried out in an Indian context. The crew at Traveling Pain School have been an integral part of it in the past. This year too we will be at Mumbai. We caught up briefly with Dr. Kailash Kothari, Organizing Secretary of ISSPCON 2014.

dear friends presenting a unique case .we had a 55yr /F admitted with pain on walking associated with perineal numbness on walking for 5-10 min,relieved at rest.she had h/o slipping of chappal from left foot since a yr.typically complaints that whenever she micturates she automatically defeacates since last 1 year but increased in severity and frequency since last 2 month at time of admission her frequency was  15-16/day.

Many times I asked about what is the ideal time for prolotherapy. nature has his own healing time of 6 weeks so after 6 weeks of injury we don't get adequate relief that is the time to initiate prolotherapy.  best example of healing is when we have fractured bone.

The patients with neck pain on one or both sides with clinical symptoms suggestive of facet joint as pain generator is very common. Many times it is being referred as spondylosis of the cervical spine

I saw 2 patients, similar complaints with severe agonizing pain in the back. They had history of cancer of prostate, operated, taken Chemotherapy few cycles and were doing well. One fine day suddenly pain in back started and it became unbearable. MRI showed pathological fracture with bone marrow edema. One patient had fracture at L2 and other one had this at L4 vertebra. We advised them VERTEBROPLASTY as immediate treatment to stabilise their spine, avoid further reduction in height of vertebra and instant pain relief.
